Who needs home health?
You may be a candidate for home health services if you’ve experienced a recent decline in your ability to perform daily tasks like getting dressed or walking. Frequent falls, recent hospitalization or the onset of a new illness can also signal the need for additional care. Difficulty leaving your home due to pain, limited mobility, shortness of breath or balance issues may also be indicators. Before reaching out to us, consult with your medical care provider to determine if home health services are appropriate. Be sure to have the necessary paperwork, such as prescription or referral, ready to help streamline the process. A medical professional referral is required to begin services.
How to become a patient
If you are looking to use Goodwin Home Health services, you will need to submit a physician’s referral. Please include the following documentation with all home health referrals.
- Patient demographic sheet with patient’s full name, address where they receive home care services and insurance information
- Referring physician’s name and phone number
- Physician order for needed home health services
- Patient’s recent physician’s note, history and physical or hospital paperwork
